Lineup Card Procedures

The league requires each team’s coach (8U, 10U, 12U and 14U divisions) to fill out a Line-Up Card and hand it to the referee at check-in. The referees use the Card to record player participation, goals scored and general conduct of the players and coaches. 

Be sure to observe the following points when you complete your Line-up Card:

  1. Fill in the top of the card. Circle whether you are the home or away team. The home team is always listed first on the schedule.
  2. List the players numerically by jersey number.
  3. List the players full names legibly, last name first.
  4. List all players on the roster, even if some are absent or unable to play.

You should tell the referee which players, if any, are absent or otherwise unable to play at check-in. Also, tell the referee which players will be sitting out as substitutes during the first period of play. The referee will make the appropriate notations on the card. Please do not mark them ahead of time.

Make a substitution plan on a separate sheet of paper in advance. You may not have time once you are at the playing field to copy the Lineup Card to a separate sheet.

There are two Hillsborough options for Line-up Cards for 2019. Both say HILLSBOROUGH LINEUP CARD. Do not use old PDFs or PDFs that you find online, even if they are from other AYSO websites. Do not use the Print Team Line Up function on the Team Roster page. 


1. Printable Line-up Cards  

This option allows you to enter your player roster directly on your computer. You can print multiple copies and have all the cards you need for the season with a few minutes effort. The printable Line-up Cards consist of two parts:

a. pre-printed perforated card stock that is blank on one side and contains the Referee Report on the reverse,
b. a PDF template with form fields that you print at home onto the card stock.

The PDF template is available here.

The card stock is available in the plastic field box in the Technical Area at each field. Please don't take more than you need.

VERY IMPORTANT: You cannot use the PDF template by itself, or with any paper or stock except the official Hillsborough perforated printing stock.

You can edit the PDF template directly using:

1. Mac System Viewer
2. Adobe Acrobat on Mac or PC
3. Google Chrome on Mac or PC (right-click on the file and select Open with Google Chrome)

which should cover most people. To edit a field just click on it. Home/Away are checkboxes. All the other fields are text entry that you can tab between. 

You have to manually enter the data for both cards in the template.
If, by chance, someone reading this is a PDF jockey and knows how to link form fields so data entered in one field auto-populates another please email the Ref Administrator and we'll update the template to make it even easier.

We recommend you enter your coach and team information for both cards in the template but keep the game-specific info blank. Then you can print it multiple times and hand-write the specific game info (Date, Time, Home/Away) for each game. 

2. Write-by-Hand Pre-Cut Card

You can hand-write your roster on the pre-printed cards which are available in the plastic field box in the Technical Area at each field. 

You must complete the Line-up Card before check-in to avoid delaying the start of your match, which becomes increasingly important as we lose daylight. 

Illegible cards or those not in order of jersey number really slow down substitutions and steal playing time from the kids. 

Please respect the efforts of the referee volunteers and maximize playing time by having a proper Line-Up Card prepared in advance. 

If you do not have a proper Line-up Card at check-in you will be asked to fill in a blank card from the field box.
